29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Jobsite-On or about November 7, 2017 employees were allowed to work from the second tier tubular welded frame scaffolding with exposed open sides and ends in that the employer failed to ensure fall protection was provided such as, but not limited to, guardrails, exposing employees to fall hazard up to 14 feet.
