29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a)  Scaffold at front-face of structure;  On or about November 7, 2017 and at times prior thereto, the employer exposed his employees to fall hazards up to 15 feet 3 inches to the ground below in that employees were allowed to climb up the side supports of a scaffold to access the 2nd buck and outrigger without the use of a ladder or other means of safe access.
