29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   A) East side of building, on or about November 8, 2017, a wooden pallet being used on a JLG extendable boom forklift to lift an employee in the air was not secured to the forks, exposing the employee to a 12 foot fall hazard.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   A) East side of building, on or about November 8, 2017, an employee was working from a wooden pallet on a JLG extendable boom forklift without guardrails or the use of a personal fall arrest system, exposing the employee to a 12 foot fall hazard.
