29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the south side of the construction site, employees engaged in in roofing activities on a pitch roof of 8:12 without fall protection were exposed to a fall hazard of approximately 11 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On the south side of the construction site, employees using an extension ladder that did not have the side rails extended at least 3 feet over the edge of the roof were exposed to fall hazards of greater than 6 feet.
