29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye protection when exposed to eye hazards from flying particles:   The employer is failing to ensure that all employees exposed to eye injuries while using pneumatic nail guns are utilizing eye protection.  This most recently occurred on November 14, 2017 on the jobsite located at 7742 Greenleaf Rd, La Vista, NE 68128.  The owner, William Cook, and two employees were observed using hand-held pneumatic nail guns to install roofing material without any form of protection to prevent eye injuries associated with flying particles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on November 14, 2017 on the jobsite located at 7742 Greenleaf Rd, La Vista, NE 68128.  Three employees were observed on the roof of a 2-story house with a 6 in 12 steep sloped pitch at heights greater than 6 feet above the ground and not utilizing any form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof.  This most recently occurred on November 14, 2017 on the jobsite located at 7742 Greenleaf Rd, La Vista, NE 68128.  Employees are using an aluminum extension ladder to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about 10 feet above the ground.
