29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On November 15, 2017 at 60 W. Pike St. Fayetteville, a convenience store building and gas station, a roofer was exposed to struck-by hazard from the lack of eye protection being used when using a pneumatic nail gun to install shingles on the roof.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On November 15, 2017 at 60 W. Pike St., a convenience store building and gas station, a roofer was exposed to a 10' 6" ft. fall hazard while installing shingles on a 5/12 pitch roof without utilizing any fall protection.

On November 13, 2017, at 60 Pike St. Fayetteville, a convenience store building and gas station, a roofer was exposed to a 10 ft. 6 in. fall hazard on a 5/12 pitch roof due to fall protection not being installed prior to the start of work which necessitated fall protection.
