29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  Building 2: On or about November 15, 2017, an employee without fall protection was exposed to fall hazards, while working from an aerial lift to load roof material.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Building 2:  On or about November 15, 2017, employees without fall protection were exposed to fall hazards greater than 6 feet to the ground below, while installing the roof of a residential building.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about November 15, 2017, employees were exposed to fall hazards greater than 6 feet to the ground below, while installing the roof of a residential building without being trained to recognize fall hazards.
