29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a)  Scaffold at face of structure;  On or about November 7, 2017 and at times prior thereto, the employer exposed his employees to fall hazards of up to 21 feet 3 inches to the ground below in that employees were allowed to climb up the side supports of a scaffold to access/egress the 2nd and 3rd bucks due to the employer failing to provide a ladder or other means of safe access.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Third buck of scaffold ;  On or about November 7, 2017 and at times prior thereto, the employer exposed his employees to a 21 feet 3 inches fall hazard in that employees were allowed to work from the third buck of a scaffold without fall protection such as but not limited to guardrail systems, personal fall arrest systems, or safety net systems.  b) Second buck of scaffold ;  On or about November 7, 2017 and at times prior thereto, the employer exposed his employees to a 14 feet 5 inches fall hazard in that employees were allowed to work from the second buck of a scaffold without fall protection such as but not limited to guardrail systems, personal fall arrest systems, or safety net systems.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Roof top;  On or about November 7, 2017 and at times prior thereto, the employer exposed his employees to an 11 feet fall hazard in that employees were allowed to work from a roof without fall protection such as but not limited to guardrail systems, personal fall arrest systems, or safety net systems.
