29 CFR 1926.403(h): Identification of disconnecting means and circuits. Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. These markings shall be of sufficient durability to withstand the environment involved.  Dorms 1352 and 1353: Four circuit breakers supplying 208 volts to a circuit were not labeled to identify its purpose and employees were installing a HVAC chiller 208 volts.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Dorms  1352 and 1353: An electrician had not been re-trained on the recognition and avoidance of unsafe working conditions while performing work to install 3 phase 208 volts three wires to HVAC Chillers.
