(b)  Machine Shop:  On November 17, 2017 and times prior, the employer did not ensure that machine guarding was provided on Vanguard vertical turret lathe (serial #M1215002, model V2522) to protect employees from the 88 inch rotating chuck.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a)  Facility Wide:   On November 17, 2017 and times prior, the employer did not conduct an annual periodic inspection between an inspector and authorized employee to ensure that the employee(s) followed the energy control procedures for the, but not limited to, the Betts 120 inch vertical turret lathe, and the requirements of this standard.
