At the worksite - On or about November 16, 2017, employees operated portable Porter Cable screw guns to secure metal roof panels to the roof without the use of eye protection, thus exposing employees to struck-by hazards.

Adjacent to the driveway - On or about and times prior to November 16, 2017, the side rails of a portable extension ladder were not extended at least three (3) feet above the roof's landing surface, thus exposing employees to a fall hazard.
