29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) At the ministry building jobsite: On or about November 17, 2017, employees were exposed to fall hazards up to 9 ft. 3 in. on a 6:12 pitch commercial roof, in that workers engaged in securing metal panels without fall protection.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards: a) At the ministry building jobsite: On or about November 17, 2017, and times prior thereto, employees were exposed to related safety hazards on a commercial roof, in that affected workers were not trained on the nature of hazards, and correct procedures to minimize those hazards as they used a 24-ft. portable ladder to access and egress the roof.
