29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):        (a)    Phase 7 Remodeling, Inc., at 12431 Maria Circle, Broomfield, CO 80020:  On and before November 17, 2017, the exposing employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that, employees decking the roof trusses on a single level residential addition were not utilizing a fall protection system.  This condition exposed employees to a fall hazard of approximately 10 feet and resulting in a serious injury to an employee that fell from the roof.
