29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:    a.  On or about August 22, 2017, at a parking lot, an employee working from an elevated basket of an aerial lift truck and did not wear a body harness and lanyard attached to the boom or basket and was exposed the falls greater than 4 feet to a lower level.
