29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     At the new residential construction site, employees used ladders that were not extended at least 3 feet above the upper landing to access the working area exposed to falling more than 18-feet from the top plates.
