29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) On or about November 28, 2017 and at time prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Jet drill press, model no.JDP-15M, serial no. 354165  was operated in the fabrication department at 4230 14th Avenue North, Fargo, North Dakota.        (b) On or about November 28, 2017 and at time prior to that date, for the employees exposed to caught-in and/or struck-by hazards where a Giddings & Lewis, radial arm drill, model no. 954, serial no. 00162 was operated in the fabrication department at 4230 14th Avenue North, Fargo, North Dakota.         (c) On or about November 7, 2017 and at time prior to that date, for the employees exposed to crushing hazards from the gantry bridge on the MG Industries, Plasma Machine, model no. TMC4514P, Serial No. 93-2503 while operated in auto mode the fabrication department at 4230 14th Avenue North, Fargo, North Dakota.

(b) On or about November 28, 2017 and at time prior to that date, for the employees exposed to amputation hazards where the Accurpress, Hydraulic Press Brake, model no. 717512, serial no. 4034 was operated in the fabrication department at 4230 14th Avenue North, Fargo, North Dakota.                       (c) On or about November 27, 2017 and at time prior to that date, for the employees exposed to amputation hazards where the Accurshear, Hydraulic Metal Shear, model no. 625010, serial no. 2222 was operated in the fabrication department at 4230 14th Avenue North, Fargo, North Dakota.
