29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems.    On or about 28 November 2017, and at times prior thereto, two employees performed roofing activities for the boat docks being manufactured or repaired without fall protection, exposing them to a fall height of up to approximately 13 feet to the ground below.

On or about 28 November 2017, and at times prior thereto, employees working at the ground level were not provided with head protection while other work was ongoing above, exposing the ground-level employees to the hazard of being struck by falling materials, tools, or debris.

On or about 28 November 2017, and at times prior thereto, the employer utilized a job-made walk board/platform that was attached to a Pettibone Rough Terrain forklift that had not been approved for use by the manufacturer, exposing employees to the hazard of being struck by or falling from the walk board/platform.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use, in accordance with the requirements of 29 CFR 1910.27(a).    On or about 28 November 2017, and at times prior thereto, a job-made walk board / platform was lifted by a Pettibone Rough Terrain forklift and used as a scaffold for elevated work without the prior approval of the manufacturer for the attachment, exposing employees working from the scaffold to the hazard of being struck by or falling from the scaffold.
