29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about November 30, 2017, at the above addressed job site, employees were exposed to a fall hazard of about 15 feet while performing roofing repairs on an elevated surface, without any means of conventional fall protection.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: On or about November 30, 2017, at the above addressed job site, employees were exposed to a fall hazard of about 15 feet while the employee who was designated as the Safety Monitor was using a leaf blower while on a lower roof area, and was facing away from the other exposed employees, who were performing roof repairs without any means of conventional fall protection.
