29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:  (a)  Alvarado Construction, LLC at 4901 W. 93rd Ave., Westminster, CO 80031:  On and before November 30, 2017, the employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, or personal fall arrest systems.  Employees were shingling a two story apartment complex with steep roofs, and were not using fall protection.  This condition exposed employees to fall hazards of approximately 20 feet.
