29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions:    a)  Interior of old Lowe's Express - Employee drove the front left wheel of the Model L2034E Simon Aerials Scissor Lift, sr #EDBO1767, into an approximate 12 inch x 12 inch x 4-6 inch deep opening in the concrete floor, causing the lift to topple over, on or about 11/30/2017.      NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
