29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about November 28, 2017: a. On the central north side of a single family dwelling where employees were exposed to a fall hazard when accessing the roof area from an extension ladder that did not have the side rails extended at 3 feet above the upper landing. b. On the northwest side of a single family dwelling where employees were exposed to a fall hazard when accessing the roof area from an extension ladder that did not have the side rails extended at 3 feet above the upper landing. c. On the south side of a single family dwelling where employees were exposed to a fall hazard when accessing the roof area from an extension ladder that did not have the side rails extended at 3 feet above the upper landing.
