(A) On Dec. 1, 2017, located on a commercial elevator shaft structure, there were no frequent and regular inspections being performed by a competent person to ensure the jobsite, materials and equipment such as tubular welded frame scaffolds & fall protection systems and/or devices were being maintained and utilized in a safe condition. (B) On or about Nov. 27, 2017, located on a commercial elevator shaft structure, there were no frequent and regular inspections being performed by a competent person to ensure the jobsite, materials and equipment such as tubular welded frame scaffolds & fall protection systems and/or devices were being maintained and utilized in a safe condition.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, shall not be used on top of scaffold platforms to increase the working level height of employees. (A) On Dec. 1, 2017, located on the south side of a commercial elevator shaft structure, an employee was observed standing on top of a 5-gallon bucket while performing masonry work on a scaffold working level platform. Potential fall heights were measured to be greater than 25 feet above the ground below. (B) On or about Nov. 27, 2017, located on a commercial elevator shaft structure, an employee was observed standing on top of blocks while performing masonry work on a scaffold working level platform. Potential fall heights were measured to be greater than 25 feet above the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. (A) On Dec. 1, 2017, located on the south side of a commercial elevator shaft structure, an employee was observed hanging off the crossbraces of a scaffold working level platform while attempting to remove some wires. Potential fall heights were measured to be greater than 25 feet above the ground below. (B) On or about Nov. 27, 2017, located at a commercial elevator shaft structure, an employee was observed hanging off the frame of a scaffold working level platform. Potential fall heights were measured to be greater than 25 feet above the ground below.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. (A) On Dec. 1, 2017, located on the east and west side of a commercial elevator shaft structure, employees were observed working from a tubular welded frame scaffold platform that did not have guardrails installed on four open sides. Potential fall heights were measured to be greater than 25 feet above the ground below. (B) On or about Nov. 27, 2017, located on a commercial elevator shaft structure, employees were observed working from a tubular welded frame scaffold platform that did not have guardrails installed on the open sides. Potential fall heights were measured to be greater than 25 feet above the ground below.
