29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees. (a) Area B (In front of the new section of the church) - On or about November 28, 2017 employees were exposed to a struck-by hazard due to the operator of a Genie GTH-1056 forklift serial no. 10137780 not wearing his seatbelt while driving on uneven earth. There were two employees on a roof in front of the forklift handling metal decking.

29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer shall not be exceeded. (a) Area B (In front of the new section of the church) - On or about November 28, 2017 employees were exposed to a fall hazard while riding in a Genie Lift S-40 serial no. 10191539 due to an exceeded occupancy capacity. This aerial lift has a warning sticker on the front that reads Maximum of 2 Occupants or 500 lbs. All three of these riders were elevated 18 feet above the ground. The operators manual also states maximum capacity of 2 occupants.
