(a)  Area B (In front of the new section of the church) - On or about November 28, 2017 employees were exposed to a struck-by hazard due to the operator of a Genie GTH-1056 forklift serial no. 10137780 not wearing his seatbelt while driving on uneven earth.  There were two employees on a roof in front of the forklift handling metal decking.

(a) Area B (In front of the new section of the church) - On or about November 28, 2017 employees were exposed to a fall hazard while riding in a Genie Lift S-40 serial no. 10191539 due to an exceeded occupancy capacity. This aerial lift has a warning sticker on the front that reads Maximum of 2 Occupants or 500 lbs. All three of these riders were elevated 18 feet above the ground. The operators manual also states maximum capacity of 2 occupants.
