Instance B:  Employees on a carpenters bracket scaffold was not adequately protected against a fall hazard of approximately 19.5 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Jobsite:  pump jack scaffold at rear of building.    A metal extension ladder used to access a scaffold work platform did not extend at least 3 feet above the upper surface of the platform.  An employees descending the ladder was seriously injured when he fell from the ladder which slipped off the work platform.
