On or about 11/02/2017, at the job site - employees erecting concrete block walls were exposed to a 12 feet 8 inch fall hazards, in that, the scaffold was not fully planked.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a. On or about 11/02/2017, at the job site - employees were exposed to a 12 feet 6 inch fall hazards, in that, the scaffold was the scaffold was missing the top and intermediate guardrails.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a. On or about 11/02/2017, at the job site - employees working from the scaffold lower levels and from the ground were exposed to head injured created by struck by hazard from falling objects such as tools and concrete blocks in that, hard hats were not provided or the use was not required by the employer.
