On or about 11/02/2017, at the job site - employees were exposed to a 12 feet 6 inch fall hazards, in that, the scaffold was the scaffold was missing the top and intermediate guardrails.

On or about 11/02/2017, at the job site - employees working from the scaffold lower levels and from the ground were exposed to head injured created by struck by hazard from falling objects such as tools and concrete blocks in that, hard hats were not provided or the use was not required by the employer.
