29 CFR 1910.107(c)(4): Electrical wiring and equipment did not conform to the provisions of this paragraph and were not otherwise in accordance with subpart S of this part (Reference: 29 CFR 1910.307, Hazardous Locations): a) Top Shelf Cabinets, Paint Booth: On or about December 5, 2017 and at times prior, employees were exposed flash fires and related secondary fires while applying flammable and combustible liquids such as, but not limited to, n-Butyl acetate, Ethyl alcohol, Acetone, Isopropyl Alcohol, Isobutyl alcohol, 2-Hydroxy-4-n-octoxybenzophenone, Toluene, Ethylbenzene, Formaldehyde(Valspar, NUF-4801, Valtec Ultra Conversion Lacquer - Matte) to finished cabinets in spraying areas while using unapproved electrical wiring such as, but not limited to, switches, lights and outlets. The spraying application occurred where combustible vapor and residue was present.

29 CFR 1910.107(c)(5): Electrical equipment not approved for locations containing both deposits of readily ignitable residues and explosive vapors was present in spraying area(s): a) Top Shelf Cabinets, Paint Booth: On or about December 5, 2017 and at times prior, employees were exposed flash fires and related secondary fires from accumulating deposits of combustibles present such as, but not limited to, n-Butyl acetate, Ethyl alcohol, Acetone, Isopropyl Alcohol, Isobutyl alcohol, 2-Hydroxy-4-n-octoxybenzophenone, Toluene, Ethylbenzene, Formaldehyde(Valspar, NUF-4801, Valtec Ultra Conversion Lacquer - Matte) while using unapproved electrical equipment such as, the unapproved radio, flexible cords and cables, and temporary lighting. The spraying application occurred where combustible vapor and residue was present.
