29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about December 6, 2017, employees were exposed to being struck by flying objects. The employees were performing roofing activities on a residential home located at 6325 Paderbone, Hudson, Ohio 44236. The employees were using air powered nail guns without wearing safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about December 6, 2017, employees were exposed to a fall of approximately 20 feet. The employees were performing roofing activities on a residential home located at 6325 Paderbone, Hudson, Ohio 44236. The employees were working on all areas of the roof without using any form of fall protection.

On or about December 6, 2017, employees were exposed to a fall of approximately 10 feet. The employees were performing roofing activities on a residential home located at 6325 Paderbone, Hudson, Ohio 44236. The employees were using extension ladders to access the roof that did not extend at least three feet above the landing area.
