Employees were observed operating Toyota and Nissan forklifts during truck loading, unloading, and warehouse operations without fastening their seat belts.

29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating: Location: 13870 Corporate Woods Trail, Bridgeton, Missouri The employer failed to ensure that removable LP-Gas containers were securely mounted on powered industrial trucks to prevent displacement. Powered industrial trucks were permitted in operation without the necessary cylinder indexing pin, without the cylinder properly seated on the indexing pin, and without the swing-open type LPG cylinder mount locked securely.

29 CFR 1910.178(l)(3)(i)(M): Any other operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee was being trained to operate. Location: 13870 Corporate Woods Trail, Bridgeton, Missouri At the time of the inspection, the employer did not ensure their employees received training on all operating instructions, warnings, or precautions listed in the operator's manual for the Nissan forklifts in use at the facility. The training did not include the following warnings listed in the operators manual: a) On the front of the manual, Warning: Do not operate the truck unless you have reviewed and understand the Operator's Manual; b) On page 5, Warning: The unit should not be operated or stopped immediately if the lamp is on. Always check that the LPG cylinder bracket is locked into position; c) On page 75, Warning: Wear protective equipment (i.e. safety glasses, gloves) and Always wear protective equipment when handling LPG cylinders; d) On page 77, Warning: Please wear gloves in order to prevent any propane vapor from contacting the skin; e) On page 78, Warning: Make sure the lock lever is locked securely.
