On or about December 4, 2017, during the installation of roofing material at the job site located at 226 Barrington Pl., Aurora, Ohio 44202; the employer did not ensure employees working on the roof of a multi-unit residential structure were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
