29 CFR 1910.95(m)(2)(ii): The employee audiometric test records did not include at least the employee's name and job classification, the date of the audiogram, the audiogram, the examiner's name, the date of the last acoustic or exhaustive calibration of the audiometer, and the employee's noise exposure assessment result:    (a) Poormans Welding and Fabrication, Inc. - The employer's audiometric test records for calendar year 2017 did not contain the employee's job classification and the noise exposure assessment result, on or about December 7, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
