This was most recently documented at 1906 Mayflower St, Bellevue, Nebraska, on or about December 5, 2017 when employees were observed using pneumatic nail guns but were not using eye protection.

This was most recently documented at 1906 Mayflower St ,Bellevue, Nebraska on or about December 5, 2017 when employees were observed installing felt paper to a residential structure while working greater than 6 feet above the ground without fall protection.
