29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  Employees engaged in maintenance activities to include changing of hydraulic hoses on the conveyor belt assembly machines in the main warehouse production area of the shop were exposed to hazardous energy in that specific control procedures to isolate equipment was not provided.  On all three machines listed below the energy control procedures did not identify specific sources of energy, individual steps to complete lock out or identify location of circuit breakers.   1) Charger Belt Slitter, Asset #54642   2) Belt Punch, Asset #054650    3) Chain Gear Belt Slitter, Asset #54643

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    Employees engaged in the production of conveyor belts using the Belt Punch machine located in the main warehouse production area of the shop were exposed to puncture hazards from the dies in that machine guarding was not provided to protect the operator and other employees.

29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection.    Employer did not conduct a periodic inspection of the energy control procedures of equipment in the main warehouse production area of the shop. Employees were exposed to hazardous energy in that the procedural requirement of this standard were not being followed at least annually.    On all three machines listed below the energy control procedures did not identify specific sources of energy, individual steps to complete lock out or identify location of circuit breakers.     1) Charger Belt Slitter,  Asset #54642   2) Belt Punch, Asset #054650     3) Chain Gear Belt Slitter, Asset #54643      Lewis Goetz and Company Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CRF 1910.147(c)(6)(ii) which contained in OSHA inspection #1086065, Citation #02, Item #01  issued on 12/21/15 (Final Order 1/20/16), with respect to a workplace located at 2113 S. West St. Suite 200, Wichita, KS.
