29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net s(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)      On the east side of the commercial building, employees engaged in roofing activities on a pitch roof of 4:12 without fall protection were exposed to a fall hazard of approximately 11 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.       On the east side of the commercial building, employees used an extension ladder that did not have the side rails extended at least 3 feet above the edge of the roof exposing the employees to fall hazards of greater than 6 feet.
