The condition was most recently observed: a. 125-127 Waterside Crossing Drive, St. Peters, Missouri when employees were performing residential roofing activities without the use of any fall protection system on a 8:12 slope roof with an eave to ground distance of approximately 9 feet, 5 inches; b. 129-131 Waterside Crossing Drive, St. Peters, Missouri when an employee was performing roof tear-off activities without the use of any fall protection system on a 6:12 slope roof with an eave to ground distance of approximately 9 feet.
