29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) On or about December 4, 2017 and times prior at 5005 Montgomery Hwy, Dothan, Al: The employer failed to ensure the East stairway was maintained free of debris which exposed employees to slip/trip/fall hazards.  b) On or about December 4, 2017 and times prior at 5005 Montgomery Hwy, Dothan, Al: The employer failed to ensure the hallways on the 2nd, 3rd, and 4th floors were maintained free of debris such as but not limited to pieces of wood, conduit, excessive saw dust, and  plumbing material which exposed employees to slip/trip/fall hazards.  c) On or about December 4, 2017 and times prior at 5005 Montgomery Hwy, Dothan, Al: The employer failed to ensure bolts sticking up and capped 3 1/2 inches throughout the hallway on the 2nd floor were removed which exposed employees to slip/trip/fall hazards.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) On or about December 4, 2017 at 5005 Montgomery Hwy, Dothan, Al: The General Contractor acting as the controlling employer failed to ensure that (Audet Electrical), a sub-contractor who's employees used the stairway on the East end of the building were protected from falls between the ground and first floor in that no stair rail system (guardrails) on the open sides were installed when they were rebuilt over the weekend and before being used.   b) On or about December 4, 2017 at 5005 Montgomery Hwy, Dothan, Al: The General Contractor acting as the controlling employer failed to ensure that (American Fire Protection), a sub-contractor who's employee used the stairway on the East end of the building were protected from falls between the ground and first floor in that no stair rail system (guardrails) on the open sides were installed when they were rebuilt over the weekend and before being used.
