29 CFR 1910.22(d)(1): (1) Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition:    a) Facility, 1000 Commerce Circle, Trafford, PA:  On or about December 8, 2017, three employees were walking on several roofs to look for leaks and one employee fell through an 18" x 18" fiberglass patch/cover that was over a previous opening and fell 30-40 feet to the concrete floor below.  The cover had not been detected from the inside because the roof had not been inspected and maintained prior to the employees going on it.
