29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed. On or about December 13, 2017 at the rear of the building, the employer allowed the employee to utilize a A-Frame ladder that was not extended out for stability and the footing was not leveled. The A-Frame ladder was closed, leaning against a wall, while being used as a non-self-supporting work platform.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents. On or about December 13, 2017, behind the building, facing West. The employer allowed the employee to utilize a cordless Hilti Drill (SFH 18-A) to drill holes in the building to attach strapping for gas pipes.
