29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Instruction on the requirements in the OSHA standard(s) for preventing injury from trench collapse, including benching/sloping methods and protective systems, was not provided to each employee working inside a trench of 7 1/2 foot depth out front of a single-family house at 199 Jordan Rd in Colonia NJ. Employee(s) were exposed to cave-in hazard due to failure to bench, slope, or shore the vertical soil walls; or to provide protective shield system such as trench box. Condition(s) were noted on or about 11/29/2017.
