29 CFR 1926.554(a)(6): All overhead hoists in use shall meet the applicable requirements for construction, design, installation, testing, inspection, maintenance, and operation, as prescribed by the manufacturer.    a.  On or about December 13, 2017, at the above addressed jobsite, employees were lifting wheel barrels filled with mortar using a Gemini Plus hoist.  Proper counter weights, were not installed on the hoist, thereby exposing the employees to struck-by materials and equipment.    b.  On or about December 13, 2017, at the above addressed jobsite, employees were lifting wheel barrels filled mortar using a Gemini Plus hoist.  The cable being used to bring the material up was damaged, thereby exposing employees to stuck by hazards.
