29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a- Building 4, 529 Woodland Road, Westampton NJ: Company employees were exposed to fall hazards, in excess of 28 feet to the ground surface below, when performing roof sheathing installation and other related activities while walking and working from a low pitched roof without the use of fall protection.  Condition noted on or about 12/12/17.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.602(c)(1)(vi): 29 CFR 1926.602(c)(1)(vi):Section 603E Industrial truck(s)  American National Standards Institute, B56.1-1969, Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed:  a- Building 4, 529 Woodlane Road, Westampton NJ: On or about 12/12/17 workers, performing plywood sheathing installation from a commercial structure's third story roof, were retrieving the new sheets of plywood from the elevated forks of an unattended Pettibone All-Terrain fork truck. As such these employees were exposed to struck by and crushing hazards.
