One worker had a harness and lanyard but was not tied off. The employees were exposed to a potential fall hazard of approximately 80 ft. to the ground below; on or about 12/19/17. b An employee was observed collecting wood on the 6th floor at an unprotected edge thereby exposing the worker to a potential fall of approximately 60 ft. to the ground below; on or about 12/19/17. c.Employees were observed using a stairway that had a landing that was missing a guardrail system at the unprotected edges thereby exposing employees to a potential fall of 10 ft. to the ground below on or about 12/19/17. d.An employee was observed using a ladder on the 1rst floor at or/and near an unprotected edge thereby exposing the worker to a potential fall hazard of approximately 15 ft. to the ground below; on or about 12/21/17. e.Employees were observed working and/or walking near unprotected edges at the site and since sections of the guardrail system were missing at the 3rd floor the workers were exposed to a potential fall hazards of approximately 30 ft. to the ground below; on or about 12/21/17. f Employees were observed working and/or walking near unprotected edges at the site and since sections of the guardrail system were missing at the 5th floor the workers were exposed to a potential fall hazards of approximately 50 ft. to the ground below; on or about 12/21/17. g. Employees were observed working and/or walking near unprotected edges at the site and since sections of the guardrail system were missing at the 6th floor the workers were exposed to a potential fall hazards of approximately 60 ft. to the ground below; on or about 12/21/17. h. An employees was observed installing guardrail on the 2nd floor of the construction site without a Fall Arrest System. The employee was exposed to a potential fall hazards of approximately 15 ft. to the ground below; on or about 12/26/17.
