29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  a)  Pallet Nail Gun Assembly:   On or about January 25, 2018 and at times prior to, the employer exposed employees to noise dose at levels of 177.4% (equivalent to an 8 hour time weighted average TWA of 94.1 dBA) which is 1.77 times the Permissible Exposure Limit of 100% (equivalent to an 8 hour TWA of 90 dBA) while working using pneumatic nail guns. Results were calculated for a 391 minute sampling period.  The employer failed to provide an annual audiometric test to the employee.    b)  Pallet Nail Gun Assembly:   On or about January 25, 2018 and at times prior to, the employer exposed employees to noise dose at levels of 123.2% (equivalent to an 8 hour time weighted average TWA of 91.5 dBA) which is 1.45 times the action level of 50% (equivalent to an 8 hour TWA of 85 dBA) while working using pneumatic nail guns. Results were calculated for a 391 minute sampling period.  The employer failed to provide an annual audiometric test to the employee.   The White's Pallet, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95 (g)(6), which was contained in OSHA inspection number 906896, citation number 1, item number 1 and was affirmed as a final order on June 11, 2013, with respect to a workplace located at 79 Upper Creek Road, Flomaton, AL 36441.
