29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and safety monitoring system or any other combination that will protect employees from falling. Jobsite: On or about December 21, 2017, an employee was observed working by the edge of a flat roof greater than 6 feet from the ground without any type of fall protection system. This condition exposed employee to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. Jobsite: On or about December 21, 2017, a employee was observed climbing down a 20 foot extension ladder that was not extended at least 3 feet above the upper landing surface. This condition exposed employees to fall hazards.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, was not secure to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder. Jobsite: on or about December 21, 2017, employees were using an extension ladder that was not secured. The ladder was setup on a parking lot driveway where the ladder could accidentally be displaced. This condition exposed employees to fall hazards.
