29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  867 SOUTH EAGLE VALLEY ROAD, BELLEFONTE PA, ROOFTOP - Employees were installing new metal roofing on the apartment building, and the employees were not using any type of fall protection, which thereby exposed the employees to fall hazards of approximately 17 feet to the ground below, on or about December 20, 2017.    No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)  867 SOUTH EAGLE VALLEY ROAD, BELLEFONTE PA, ROOFTOP - Employees used a Werner 28 foot extension ladder to access the roof of the structure, and the rails of the ladder only extended approximately 18 inches past the edge of the roof, which thereby exposed the employees to fall hazards of approximately 17 feet to the ground below, on or about December 20, 2017.    No abatement certification or documentation required.
