29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    a) Mehadrin Dairy Corporation/Elizabeth, NJ- Warehouse/Freezer Area: The employer did not ensure that exit routes were unobstructed. A plastic wrap was placed within the exit route in front of the exit door obstructing employee access in case of an emergency.     Condition observed on or about December 20, 2017.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:      a) Mehadrin Dairy Corporation/Elizabeth, NJ- Warehouse Area: Refresher training, including an evaluation of powered industrial truck operator's performance and the effectiveness of that training was not conducted to ensure that the operator has the knowledge and skills needed to operate a powered industrial truck safely. Employees were last trained in 12/20/2013 and continued to operate powered industrial equipment such as Yale and Toyota propane powered forklifts, and Crown electric forklift on a daily basis.    Condition observed on or about December 20, 2017.
