The employer did not verify and evaluated the workplace through a written certificate that a hazard assessment was performed.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   On or about December 28, 2017, and at time prior thereto, at this precision machining and metal parts manufacturing plant, employees were involved in loading and unloading heavy metal alloys parts, equipment and materials operations and were utilized powered industrial trucks (PITs) without training and evaluation from the employer. This exposed employees to struck-by and caught-in-between hazards.
