29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Shop Area:  The employer did not ensure the use of personal protective equipment such as face shields or goggles where employees were exposed to caustics and severe irritants  such as fuel system cleaner mixed with engine fuel during vehicle maintenance resulting in splash hazards to the eyes and face on or about 12/13/17.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body was not provided within the work area for immediate emergency use:   a) Shop Area: An eyewash station, capable of 15 minutes of continuous flushing, was not provided where chemicals that can cause severe irritation were used, such as, but not limited to, fuel system cleaner, engine, and Parts wash cleaners on or about 12/19/2017.
