29 CFR 1926.501(b)(2)(i): Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about December 20, 2017, on the new 2:12 pitch roof, employees installing sheeting were not protected from a 24 feet fall hazard by they use of a fall protection system.     b. On or about December 28, 2017, on the existing 2:12 pitch roof, employees installing sheeting were not protected from a 16 feet fall hazard by the use of a fall protection system.

On December 28, 2017, on the 2:12 pitch metal roof, the skylight was not adequately covered by a secured cover, exposing an employee to a 17 feet fall hazard.
