29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Shop: On, or about, January 3, 2018, on the following machines, door interlocks had been bypassed: A. Okuma MU-500VA. B. Tsugami FMA3-II. The tool changer cover interlock had also been bypassed. C. Hitachi Seiki lathe. D. Okuma MU-400V II. Also, the panel/door provided for the tool changer area was removed from the machine. E. Turning Area, Okuma LB300EX II. F. Turning Area, Okuma Space Turn LB3000EX. G. Turning Area, Okama Space Turn LB3000EX.
