29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.        On or about January 9, 2018, the employer did not provide the employee(s) with fall protection training necessary for hazard recognition and understanding for safe work practices. This condition exposed employees to falls exceeding 10 feet.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    Construction site: On or about January 9, 2018, an employee working from an aerial lift was not wearing fall protection equipment nor a body belt with lanyard attached to the boom of the aerial lift.  The employee was exposed to a fall hazard of 10-feet or more above lower levels.    Allen Concrete Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1135079, citation number 1, item number 1, and was affirmed as a final order on April 27, 2016 with respect to a workplace located at 3960 Zaragoza El Paso, TX.
